Case Management
in Wayne & Holmes County
Life doesn't happen in an office. That's why Anazao's case management workers come to you.
Whether you're working through something at home, navigating a complicated system, or just trying to get through the week, we show up in the places where your real life happens.
You don't have to figure it out alone. You don't even have to come to us first.
Meeting You Where You Are
Some of the best support happens outside of a clinical setting. Our case management workers come to you at home, in your neighborhood, or wherever you feel most comfortable. This allows the support you receive to fit your goals and the realities of your everyday life.
Transition to Independence Process
Our approach is inspired by the evidence-based Transition to Independence Process (TIP) model. This helps people build the skills and confidence to function more independently in areas such as employment, education, housing, personal well-being, and community life.
More than a once-a-week check-in and a handout, the TIP model is practical, goal-focused support shaped by what matters to you, provided by people who are invested in helping you build a life that feels more stable and manageable.
Advocacy and Outreach
Systems are complicated. Schools, healthcare providers, courts, social services. They all have their own processes, their own language, and their own expectations. It's a lot to handle on your own.
Our case management workers can go with you. To a doctor's appointment. To a school meeting. To court. Wherever you need someone in your corner who knows how to navigate the process and speak up for what you need.
You shouldn't have to translate a complicated system by yourself. We're there to help make sure your voice is heard.
Therapeutic Support
Progress isn't always a straight line. Life gets in the way, stress builds up, and sometimes you just need someone checking in.
Our therapeutic support is about helping you stay connected to your goals, not just when things are going well, but when they're not. That might mean talking through what's been hard, working on ways to manage stress, or practicing skills that help you handle whatever comes up in daily life.
It's not about being fixed. It's about having steady support while you keep moving forward.
Daily Living Skills
Confidence in everyday life doesn't always come naturally, and that's nothing to be ashamed of. Sometimes we all need a little help building the routines and skills that make life feel more manageable.
Our case management workers help with the practical stuff: staying on top of routines, handling responsibilities, and feeling more at ease in your home and community. The goal is simple: that you feel more capable and more comfortable in your own life.
Small skills, practiced consistently, make a real difference over time.
Service Coordination
Service coordination supports children and families facing complex challenges that may put a child at risk of being placed outside the home. Our team helps connect families with the right services and support so the child can safely remain at home whenever possible.
Anazao partners with the Wayne County Family and Children First Council to provide this service. Together, we help families navigate providers, schools, agencies, and other resources while keeping everyone focused on the child’s well-being.
Families do not have to coordinate every moving part alone. We help bring the right people together, improve communication, and create a plan built around the needs of the child and family.
Whether you’re just getting started or need help navigating services, we’ll walk with you through the process.
Prevention
Prevention starts with giving people the knowledge and tools to recognize concerns early and respond with care. Anazao provides prevention programs in schools as well as training and education for professionals and community organizations.
These services may address suicide prevention and other behavioral health concerns affecting children, families, and the wider community. By helping people recognize warning signs, understand available resources, and know what steps to take, we can support individuals before a difficult situation becomes a crisis.
